Need to cancel your membership?
We’re sorry to see you go! Please fill out the form below to cancel your membership. We require a 30-day notice. If you request before the 30-days, there is a cancelation fee of $40. If you request within 90 days of your membership start date, there is a cancellation fee of $80. We hope to see you back soon!
